Membership Cancellation Request Form
To cancel your SHOP Membership, please read below for terms and conditions and fill out the form to submit your request. After filling out the form below, please allow five (5) business days for us to process your request.
FORM SUBMISSION REQUIREMENTS
- The cancellation date must be in the future (no past dates or retroactive cancellations).
- All membership payments are automatic, and we cannot issue refunds if you submit your request after a payment has already processed.
If you are on the month-to-month plan, your membership is cancelable at any time without a fee.
EARLY TERMINATION FEES
To cancel an agreement before its term has expired, the following cancellation fees will apply:
- 12-Month (or 52-Week) Contract = $240 + any promotional discount received
- 9-Month (or 40-Week) Contract = $180 + any promotional discount received
- 6-Month (or 28-Week) Contract = $120 + any promotional discount received
“NO FEE” TERMINATIONS
- If you have a medical issue that prevents you from utilizing your membership for the foreseeable future, you may be allowed to terminate without penalty by providing a note from your doctor detailing the medical necessity of the termination. If your medical issue prevents you from utilizing your membership for a certain period of time (such as injury), please fill out the Membership Pause Request Form.
- If you move more than 25 miles away from the SHOP, you may be allowed to terminate without penalty by providing proof of new residence.
